Made in the USA. I do not personally test each light bulb. However, i've been working with the same distributor for a while now and sold hundreds of bulbs without much indication failure rate. I cannot tell by the outside of cases which are.

Note: Some of the boxes may show signs wear - crushed on one side or another in several cases bulbs have been replaced with those other new 4 packs.

Assuming this, it could be a home business setting. My packing process is quite extensive - it includes wrapping each 4-pack in several layers of plastic wrap, then a complete layer bubble wrap.
